An audit of parenteral nutrition provision in acute care.

Abstract

Parenteral Nutrition (PN) is the intravenous administration of nutrition to patients who cannot meet their nutrition needs via the gastrointestinal tract either orally or enterally. At Epworth Richmond, PN is provided according to best practice clinical guidelines as part of a multidisciplinary team (MDT), primarily to support recovery in the gastrosurgical patient cohort. While previous audits have focused on logistical aspects of PN delivery, this is the first audit at Epworth Richmond to evaluate clinical outcomes. The aim of this audit was to ensure PN is used safely, appropriately, and effectively.PN use was found to be clinically appropriate and well monitored. Malnutrition was indicative of risk of refeeding syndrome, highlighting the need for early malnutrition screening and timely nutrition support. Complications such as electrolyte disturbances and line issues emphasise the value of specialised PN MDT management and staff education. Education with medical and nursing teams has since been completed. Future work aims to expand the current PN protocol to better guide the management of longer term PN patients, building stronger pathways with specialised intestinal failure clinicians and ensuring Epworth patients continue to receive excellent quality care.
